Forms needed for the ULF fundraiser are below.
Orders need to be turned in to the SAHA main office mail slot no later than Monday October 2nd, in an envelope clearly marked ULF fundraiser. DO NOT TURN IN YOUR ORIGINAL ORDER FORM, use the separate sheet to turn into SAHA.
Payment for orders must be included in the envelope. Checks should be payable to SAHA.
Orders will be delivered to SAHA mid November, and you will receive your profit check at the time of pickup.
